St. Paul, MN – July 10, 2024 – The Arc Minnesota has decided to stop providing Housing Stabilization Services (HSS) effective immediately. The waitlist for these services is now closed, and no new referrals or service requests will be accepted.
Current program participants must select a new provider through the Department of Human Services (DHS). Individuals should coordinate with their Case Managers or Housing Consultants to submit Form DHS-7948-ENG to the DHS website, accompanied by an updated Coordinated Services and Supports Plan (CSSP) or Housing-Focused Person-Centered Plan.
The updated housing plan document must include the new provider’s name and all required signatures. When completing Form DHS-7948, please select “Provider Change Request” from the “Reason for Submittal” drop-down menu.
